Have you been convicted on more than one date? Example: Guilty of Misdemeanor Larceny on September 9, 2011 and Guilty of Simple Possession of Marijuana on November 12, 2012
Have you been convicted for Possession of Cocaine, Heroin, Meth, or other serious drugs (Not Marijuana) or convicted of PWISD (Possession with Intent to Sell & Deliver), Selling, or Delivering Drugs of any Kind?
Have you been convicted of any Felony Class A through Class G?
Have you been convicted of an A1 Misdemeanor (Assault, Domestic Violence, Stalking, Violation of a restraining order)?
Have you been convicted of anything plus conviction of probation violation?
